- You want Curry Health Center to send your health records to another location.

Note for Brief Illness
Curry Health Center does not provide sick or excuse notes for brief illnesses (like colds, headaches, nausea, or other short-term issues) because these cannot be medically verified after the fact.
If your instructor asks for documentation for a minor illness and you did not visit our clinic, you may use the form below to document your absence. This form is not a medical excuse, it simply explains our clinic policy.
If you were seen at Curry Health Center, you can print proof of your visit from your secure Patient Portal if you choose.
Students should communicate directly with their instructors about missed classes or assignments.
